Terms
Informational terms for this website.
These terms cover use of the public ArkMedis marketing pages. They are not a statement of work, not legal advice, and not a promise that a review will be accepted.
Who publishes the site
The site is published by FabCare LLC d/b/a ArkMedis, FEIN 33-1873176, primary office at 396 West Greens Rd, Suite 1115, Houston, TX 77067. The public brand is ArkMedis. Phone 832-479-9378. Fax 832-678-7069 and admin@arkmedisreviews.com are non-PHI channels.
What the pages are
Content describes independent clinical review and the limits around it. It is not medical advice to a patient, not a coverage determination, and not legal advice. Reading it, linking to it, or sending an email does not create an attorney-client relationship. ArkMedis is not a law firm. See Legal services.
What the pages are not
- Not a certification, accreditation, or license map
- Not an offer of consulting, software, or workers’ compensation review
- Not a guarantee of turnaround, outcome, or panel size
- Not an invitation to send protected health information
Texas IRO certification is not active. The status callout is on For payers. Seals we do not hold are listed on Security & Trust.
Engagements are separate
A review engagement exists only when ArkMedis and the client agree in writing on scope, including the clinical question and whether we can staff the specialty. Website copy does not set a price, a deadline, or a state’s external-review authority.
Portal access
The case workspace is only for authorized users. Sign-in requires MFA. Misuse, attempts to reach another customer’s files, or sending PHI to public email or fax are outside these terms and outside the portal rules on Security & Trust.
Acceptable use
Do not misuse the site, probe it, or submit records it tells you not to submit. Trademarks and page text stay ArkMedis property. You may link to public URLs. You may not copy the site into a product or imply a partnership we have not stated.
Changes
We update pages when the facts change. The current page is the current statement. These terms are not a negotiated contract. If a written engagement conflicts with a marketing sentence, the written engagement controls that engagement — it does not expand the marketing claims.
