What this page covers
These notes describe the public pages on arkmedisreviews.com. They are not a HIPAA notice of privacy practices for a health plan, and they are not legal advice. The authenticated case workspace is a separate channel, described on Security & Trust.
Information you send us
If you call, fax, or email the office, we receive what you choose to send: typically a name, a company, a phone number, and a question. Do not include member identifiers, clinical records, or claim numbers. Email and fax are non-PHI administrative channels.
Secure case upload is available for authorized portal users (Business, Provider, and Admin) after MFA: a verified email and a second factor, with an authenticator preferred and US SMS supported. ArkMedis will review and process submitted cases after Texas Department of Insurance Independent Review Organization certification is issued. The application is filed and in process. Certification is not active. Send case PHI only through the portal vault. Do not send PHI by email or fax. Download of stored files is not available yet.
What we do not run here
- No advertising pixels and no ad network
- No marketing analytics tag on these public pages
- No sale of contact information
- No account registration on the marketing pages
The site host and our email provider process ordinary technical data such as connection logs needed to serve the site and deliver mail. That is not a tracking program, and we do not claim a formal log-retention schedule beyond what those providers keep for operations and security.
The portal
Authorized users sign in with MFA. Workspace and Google Cloud HIPAA BAAs are held. Those agreements are not accreditation and not Texas Department of Insurance certification. Download of stored files is not available. Portal rules on Security & Trust control if this page and that page ever differ.
